What Is Align and Restore Treatment in Cosmetic Dentistry?

For decades, patients seeking a cosmetic makeover faced a difficult, almost unfair dilemma.

You wanted to fix your smile, maybe to hide deep fluorosis stains that whitening couldn’t touch, close a persistent midline gap, or fix a crooked tooth that ruined photos but the cost of perfection was terrifying.

To get that “Hollywood” look, dentists of the past had one main tool in their arsenal: The Crown.

To make a twisted tooth look straight, or to mask deep intrinsic discoloration, they had to grind away up to 60% or 70% of your healthy tooth structure. They turned strong, living teeth into small, peg-like stumps just to cover them with artificial caps. It was a compromise: You got a perfect look, but you sacrificed your natural teeth to get it.

The “Old School” Problem: The Geometry of Destruction

To get an idea of what Align and Restore is, let’s first discuss the shortcomings of the previous technique for these types of cases.

Historically, a case of a patient with a “composite type’ smile (i.e., a smile with combined intrinsic yellow/brown staining, plus crowding) presented a very complex geometrical problem for dentists when they attempted to restore these cases.

Dentists could never adhere a thin veneer to a crooked tooth because the veneer would be improperly positioned and appear too thick. Dentists had no option but to “subtract” tooth structure interfering with placement of the veneer in order for them to restore the smile to look uniform.

This method is regarded as an aggressive technique as it removes enamel (the protective layer of the tooth) and exposes the dentin.

Once tooth structure is removed, there is no way to return it back to its original condition. After grinding down a tooth, a crown must be used as replacement for that tooth. If that crown fails 15 years later, the underlying tooth is already compromised with its inherent weakness and has little protection from future dental damage.

The New Solution

The Align and Restore protocol solves the geometry problem without the drill. By splitting the treatment into two smart phases, we can save your tooth structure.

Phase 1: Alignment (The “Setup”)

When we consider the shape, color, and texture of teeth we want to accomplish the desired results from an orthodontic perspective. Using clear aligners (such as Invisalign) or brackets & wires, the orthodontist repositions your teeth to their ideal architectural location.

  • Crowding – Ideally, if a person has a crooked smile the dentist is going to “un-ravel” the crowding issue to create a straight surface.
  • Spacing – if a person with gaps or spacing in the middle of their teeth would have an even spacing without any “gaps”.
  • Preparing for Peg Lateral Teeth – If a person has very small peg lateral teeth, the dentist will create enough “space” around the peg to build them up later without putting excessive pressure on the surrounding teeth.

By completing these two phases, there are no longer any “fake” dental connections created. We now have an optimal foundation to work on for a lifetime.

Phase 2: The “Restore” Toolkit (Preservation First)

Because your teeth are now straight, we can be incredibly conservative. We no longer need thick porcelain to hide crookedness. We can choose the tool that requires the least amount of touching your natural tooth.

1. For Shape Issues (Peg Laterals & Gaps): “No-Prep” Veneers

Many patients have “peg laterals”—where the side teeth are naturally small or pointy.

  • The Old Way: Grind the small tooth even smaller to fit a crown.
  • The Align & Restore Way: Since the tooth is already small, we simply add to it. We use No-Prep Veneers. We bond a thin ceramic shell directly onto the enamel without drilling a single millimeter. We are building volume, not taking it away.

2. For Color Issues (Fluorosis): Minimal-Prep Veneers

Deep intrinsic stains from fluorosis (stubborn white or brown bands) sit deep inside the enamel. They can’t be whitened, but they can be masked.

  • The Old Way: Thick crowns to block out the color.
  • The Align & Restore Way: We use Conventional Ceramic Veneers or Composite Laminates. Because the tooth is straight, we only need to remove a microscopic layer of enamel typically just 0.3 to 0.5 mm (thinner than a fingernail). This is just enough space to layer the porcelain and hide the stain, while leaving the core of your tooth 100% strong.

3. For Minor Chips: Composite Bonding

For small irregularities on the edges or tiny chips, we use high-strength “color fillings” (composite resin) to artistically sculpt the perfect edge. It’s fast, non-invasive, and looks hyper-realistic.

The Timeline: Delayed Gratification for Lifelong Health

One question we often get is: “Why does this take longer?”

It is true. The old method of “grinding and capping” could be done in a week. Align and Restore takes time because we have to wait for the alignment phase (Phase 1) to complete. Depending on the severity of your crowding, this could take 3 to 12 months.

But we urge patients to look at the timeline of their life, not just the next month.

  • The Short Term: Yes, you wear aligners for a few months.
  • The Long Term: You keep your natural enamel for the next 40 or 50 years.

A tooth that has not been ground down is significantly less likely to need a root canal, less likely to fracture, and less likely to fail than a crowned tooth. The few months you invest in alignment pays dividends in decades of oral health.

The “Crown” Exception: When Do We Use Them?

You might wonder, is the crown dead? Not entirely. But in the Align and Restore philosophy, a crown is treated as a medical device, not a cosmetic one.

We reserve full-coverage crowns strictly for teeth that are already compromised:

  1. RCT Teeth: A tooth that has had a Root Canal Treatment is dehydrated and brittle. In this scenario, a crown is essential to act as a “helmet” to prevent the tooth from shattering under bite pressure.
  2. Heavily Damaged Teeth: If a tooth has a massive cavity that has destroyed more than 50% of the structure.

However, we do not crown healthy teeth just to change their color. That is a violation of the Align and Restore ethic.
